Rust 1.66.0 was announced[0].
The code fixes were all for the clippy warning regarding needless
borrows.
The doc tool now parses for HTML, so any <placeholder> now needs to
be wrapped in backticks, i.e. `<placeholder>`.

Reorganise the tests into module files, rather than all being in a
single file. No tests were changed in this patch.
The `serialize` flag was also not enabled in `test/Cargo.toml`. This
is enabled to ensure those tests are run moving forward.
